The "Day Break" Adult Day Health Care service provided by Castle Family Health Centers at our Bloss site located at 1251 Grove Avenue, in Atwater, CA, provides an organized day program of therapeutic, social and health services for the frail elderly, and other adults with physical or mental impairments.

Any adult is eligible who meets all of the following requirements:

  • Has a medical condition that requires treatment or rehabilitation prescribed by a physician
  • Has a physical and/or mental impairment that limits the ability to carry out the activities of daily living (like dressing, cooking, cleaning, etc.) but is not yet so severe as to require 24-hour care
  • Out services will maintain or improve the client’s present capabilities
  • Is likely to suffer further deterioration and probable require 24-hour institutional care if Adult Day Health Care services were not available

Clients can arrange to come to Day Break one to five days a week - Monday through Friday from 8:00 in the morning to 3:00 in the afternoon. Clients can enjoy our services all day, or for at least a minimum of four hours each visit. Administrative hours are Monday through Friday from 7:30am to 3:30pm.

Program Costs

Day Break costs may be covered for eligible participants by Medi-Cal, Veterans Administration, some long-term Care (LTC) insurances, and private insurance plans. If you are not covered by one of these, please contact the Day Break Medical Social Worker to discuss payment options.

What is Day Break?

Day Break is an organized day program of therapeutic, social and health services. It is a safe, medically supervised and home-like environment for:

  • Seniors
  • Physically and Developmentally disabled
  • Those with Alzheimer's
